You will need to discuss which Medicare plans LifeBridge Health will work with in 2025.  This will ensure continued access to LifeBridge Health Providers. Our licensed Medicare Broker partners will help you understand all of your plan options in 2025 and will present you with various plans, benefits and pricing and assist you in the enrollment, right over the phone.  

There is no obligation to change and there is no cost for this service.  If you agree to change plans, they will handle all the paperwork.  All brokers are based in Maryland and licensed by the state. They are compensated for their help from the insurance companies directly.  There is no cost to you.

Yes, this varies by which plan you have now.

Medicare has set strict deadlines depending on which plan you currently have for 2024. If you have a Humana PPO or HMO Advantage Plan (Gold Plus / Choice Plans), you have until December 7th, 2024 to make a change.  This will ensure your coverage will start on January 1st, 2025.

If you have an Aetna HMO Advantage Plan (SmartFit Plan), your deadline will be extended to December 31st, 2024 to make a change.  

Due to Holiday Season office hour disruptions, it is not advisable to wait too long to make these changes.

The informative call will only take 10 to 15 minutes.  

Enrollment into a new plan takes only 10 minutes.  If you discuss the options with one of our Preferred Brokers, and decide to enroll on the same call, the total call length should not exceed 30 minutes.

You will receive new membership cards for your Advantage Plan within two weeks of your enrollment.

All new plan changes will start on January 1st, 2025.
